Planning for Hospice

grieving togetherEnd of Life planning is important to make sure our wishes are fulfilled when we have a limited life expectancy. Hospice and Palliative Care means that we now want comfort care instead of curative care. We may not want CPR or extraordinary life measures if our health is failing or we have an end stage chronic disease.

We should let others know how we want to spend our final years, months, or days. Advanced Directives or Five Wishes help us explain to our loved ones what things are important to us and how we want our medical, social, spiritual, and everyday needs met during our last days of life. The goal of Hospice is to help meet those wishes. The patient and family will help the Hospice Team build a Plan Of Care to meet those needs.

When our wishes are written at end of life, our spouses, children, and grandchildren are not placed with the burden of trying to decide what we would have wanted.  Using the 5 Wishes Guide or Advanced Directives help us write our wishes for end of life.

We can spend quality time with our loved ones.

Now that we have taken care some of the end of life planning, we can return to the things that we love to do most.

    I’m 89 years old; I have good health and plan to take care of myself as long as possible. I want to be prepared to get help if needed and would appreciate any information you can send to me. I am interested in the number of hours I would have help in my own home and what the cost would be.

    I have two children who are well educated and have always taken care of themselves but they would be a help to me if I needed it.

    This is not an application for help but as much information about your program as possible.
